The Opportunity:
As a Care Navigator at Cityblock Health, you will play a pivotal role in assisting our members through their healthcare journey. You will be an advocate for members needing a connection to various healthcare services and benefits in the community. Your responsibilities will include: answering phone calls, triaging member requests, documenting interactions, performing data entry, conducting follow ups, coordinating appointment scheduling, working within internal/external care teams and building meaningful connections to members. In this role, you will report directly to the Member Experience Operations Supervisor and will work collaboratively with the Care Team including Care Managers, Nurses, Social Workers, Behavioral Health Specialists, Member Support Representatives and others. How You'll Contribute:
Serving as an advocate for members as they seek care throughout the healthcare system Responding to requests that may include assistance with health-related questions, scheduling appointments, providing education about our services, coordinating communication, arranging transportation to appointments, and coordinating medical records. Assisting in data collection, information verification, data entry, and ongoing tracking of various outreach strategies to ensure our services meet each member’s unique needs and care plans. Building strong and reliable relationships with Cityblock’s members and serving as their trusted liaison with Cityblock’s community of services and network of medical care providers. Collaborating within a fast paced work environment to proactively connect members to Care Services Communicating efficiently through various channels in alignment with workflow procedures Actively working with and learning our CRM (Cityblock's technology-enabled Care platform) Using empathy and solution-driven tools when engaging with member needs and challenges Maintaining knowledge of available health plans and options, helping to ensure Cityblock members are able to make choices that serve their individual circumstances Ensuring all contacts are fully documented using standard workflows and tools, and that member interactions are handled as sensitively as possible, honoring individual boundaries Working both independently and collaboratively as an active participant in weekly meetings, to solve any member-related problems and coordinate care Ability to stay calm, maintain perspective, and function effectively in stressful circumstances Other duties as assignedWhat You'll Bring:
1+ years of experience working in customer service 1+ years of experience working with a customer relationship management (CRM) database to manage interactions with the population Excellent phone and verbal skills required, able to easily converse with customers and explain available services to encourage service adoption. Outstanding attention to detail and accuracy and experience documenting client interactions according to HIPAA/compliance regulations Excellent follow-through, ensuring our members' needs are anticipated and requests are managed in a proactive, supportive way Ability to develop warm, friendly, and communicative relationships with diverse members A genuine interest in connecting with people in underserved communities Demonstrated passion for excellence, with a drive to learn quickly and grow personally and professionally Spanish language proficiency High School diploma or GED equivalentNice to Have, But Not Required:
Previous experience working with patients with complex medical, behavioral health and/or social needs Experience working in a healthcare call center environment. Experience with Google suite and Microsoft Office products and Slack (our internal messaging/communications system)Benefits We Offer:
